They cut it with surgical instruments then historically they would draw blood from the wound with the lips to make sure it would heal properly. Most Jews abandoned this process when there became better ways to do it. Certain orthodox sects still practice this form. There is a controversy because a Mohel had a cold sore and the virus infected the baby and it died from complications of the Herpes.
