The Red Skull had to order Cain to stop before he pummelled the Hulk to death

It wasn't a what if story, I had the comic. Juggernaut was working for the Red Skull, Hulk went to take him on and Caine Marko (Juggy) was acting as his bodyguard but was in civilian duds. Before Hulk realised who he was, the fight was over and Caine had him.
